چکیده

Pharmacologic therapy for hypertension is effective for the majo rity of patients with hypertension, but there remains a subset of the population with treatment resistant hypertension who cannot achieve their blood pressure (BP) goal despite multiple medications. For these patients at increased risk of cardio vascular disease and end-organ damage, additional therapies need to be considered. This review will cover a non-pharmacologic approach to hypertension through stimulation of the Baroreflex (Baroreflex activation therapy). While Baroreflex therapy remains investigational at this time, for patients with treatment resistant hypertension, this therapy may offer a novel option to achieve BP goals and hopefully reduce the risk of heart attack and stroke.

Carotid Baroreceptor Stimulation: A Potential Solution for Resistant Hypertension

Resistant hypertension indicates that the blood pressure cannot reach the target value despite standard drug treatment, which harbors an increased risk for cardiovascular diseases. The role of the carotid sinus in regulating blood pressure has long been observed; thereby, the idea that treating resistant hypertension by stimulating carotid baroreceptors emerged. Nevertheless, this idea has been...

Resistant Hypertension Treatment through Carotid Baroreceptor Stimulation

A significant number of hypertensive subjects fail to achieve adequate blood pressure control despite adherence to maximal doses of several antihypertensive drugs. Electrical stimulation of the carotid sinus is a new and interesting approach to the treatment of resistant hypertension. The purpose of this paper is to overview the argument starting from the historical physiological background to ...

Insights into pathophysiology of carotid baroreceptor stimulation as a method for treatment of resistant hypertension

Resistant hypertension is the term used for patients who are tolerant to a maximum of three doses of antihypertensive drugs, where one of them is a diuretic. Resistant hypertension also applies to patients who are unable to reach the target blood pressure. Patients with resistant hypertension are at a higher risk of cardiovascular morbidity and mortality than those whose hypertension is control...

Effects of chronic baroreceptor stimulation on the autonomic cardiovascular regulation in patients with drug-resistant arterial hypertension.

In patients with drug-resistant hypertension, chronic electric stimulation of the carotid baroreflex is an investigational therapy for blood pressure reduction. We hypothesized that changes in cardiac autonomic regulation can be demonstrated in response to chronic baroreceptor stimulation, and we analyzed the correlation with blood pressure changes. Twenty-one patients with drug-resistant hyper...

Role of Chemoreceptor Activation in Hemodynamic Responses to Electrical Stimulation of the Carotid Sinus in Conscious Rats.

Electric carotid baroreflex activation has been used to treat patients with resistant hypertension. It is hypothesized that, in conscious rats, combined activation of carotid baro- and chemoreceptors afferences attenuates the reflex hypotension. Rats were divided into 4 groups: (1) control group, with unilateral denervation of the right carotid chemoreceptors; (2) chemoreceptor denervation grou...

Carotid Baroreceptor Stimulation for the Treatment of Resistant Hypertension

Interventional activation of the carotid baroreflex has been an appealing idea for the management of resistant hypertension for several decades, yet its clinical application remained elusive and a goal for the future. It is only recently that the profound understanding of the complex anatomy and pathophysiology of the circuit, combined with the accumulation of relevant experimental and clinical...
